Use of PhilHealth
Identification Number (PIN) for PhilHealth transactions
For all accredited institutional health care providers(No.
08-001-2005)
In line with
Circular 25, series of 2002, we are encouraging
members to use their PhilHealth Identification Numbers
(PIN) for their PhilHealth transactions to enable
us to facilitate claims processing and to establish
and maintain an updated membership and contribution
database.
In availing themselves
of PhilHealth benefits in your institutions, members
are encouraged to use their PIN. Those who have
not been issued their PIN yet may register with
us through any of our Regional or Service Offices
nationwide to ensure expeditious processing of their
claims.
For members without
PIN, the following documents must be submitted to
your Medicare Section and attached to claims applications
for automatic inclusion in our database:
For Employed
Members:
- 2 copies of Form
M1a (Member Data Record)
- 2 copies of Form
ER1 (Employer Data Record)
- 2 copies of Form
ER2 (Report of Employee-Member)
- Employers who have
been issued their PEN need not submit the ER1.
For Retirees/Pensioners:
- 2 copies of Form
M1C (Member Data Record)
- 2 1” x 1”
ID photos
Members who wish to
update their personal data with PhilHealth must
accomplish Form M2 (Amendment) and submit this,
together with supporting documents, to any of our
offices nationwide. Forms M1a, M1c and M2 are available
in any of our offices.This advisory does not prevent
any member from availing himself of the benefits.
Eligible members may still be entitled to the benefits
through automatic deduction from hospital charges
upon discharge, even without the PIN, until such
time as may be prescribed by PhilHealth.
